For example, if you place an order for the NELLA Patient Comfort Kit (including the related medical device), we will ship the device to your health care provider, contact your health care provider by telephone or email, share your name and birthdate for identification purposes with your health care provider, and communicate with your health care provider about your wish to have your provider use the Nella speculum in medical exams and procedures.  WHEN YOU SUBMIT AN ORDER FOR A PRODUCT THAT WILL BE SHIPPED TO A THIRD PARTY (INCLUDING YOUR HEALTH CARE PROVIDERS), YOU ARE DIRECTING US TO SHARE YOUR RELEVANT PERSONAL INFORMATION AND PRODUCT WITH THAT PARTY.  WE HAVE NO CONTROL OVER THE ACTIONS OF ANY THIRD-PARTY RECIPIENTS OF SUCH INFORMATION.  ACCORDINGLY, WE CANNOT AND DO NOT GUARANTEE THAT SUCH INFORMATION WILL NOT BE ACCESSED OR USED BY UNAUTHORIZED PERSONS.
